Youth Faith Formation Registration Form
Fill out the Family/Parent page as legibly and completely as possible. Forms that we are unable to read or do not have sufficient information will not be processed and children will not be assigned. Both parent page and student page(s) must be sent in together.
Fill out a student sheet for each student as legibly and completely as possible. Number preferences (1-3) for class day & time for children in pre-school and grades 1 – 6. Preferences are honored if possible. Students are assigned to classes in the order that registrations are received. Sunday 10:15 am classes are usually filled by those who register before the end of May. If preferences are not numbered (1-3) on the form for preschool through grade 6, we will assume you have no preference. Special Needs, grade 7-8, and grades 9-12 are available only on one day and time. (Extra student pages can be printed from the download section.)
Include copies of baptismal certificates for First Communion or Confirmation preparation.
VBS Registration for Operation Overboard is going on now. Deadline is April 22nd . $10 more for forms received after 4/22. No forms will be accepted after 4/30. Dare to Go Deep with God!
Faith Formation Registration for the 2012-2013 school year begins April 1. Registration ends June 1. 2012. All registration forms received after the early discount period that ends June 1
st, 2012 must include the full registration amount of $95.00. Class assignments are first come first serve. Please register early. Class sizes and times will be based on enrollment received by June 1, 2012. If your students participated in the 2011/2012 FF school year you will automatically be mailed a registration packet to complete and return. Forms are available in the narthex or online.
If you are able to volunteer, please print and fill out the volunteer form and return it with the registration form.

Acknowledgment of registration along with you child’s class assignment will be mailed at the end of August. Classes begin on Sunday, Sept. 11 and Wednesday, Sept 14, 2011
